If you’re claiming benefits under the special rules for terminal illness (SRTI), you may be asked to send a factual statement from your doctor, consultant, Macmillan Nurse or other healthcare professional when you make the claim. This statement is called a DS1500 form. It is also known as a DS1500 medical condition report. A DS1500 form describes your illness and treatments. The form is a factual report about your condition and symptoms. If you’re applying for a benefit on someone’s behalf, you may need to ask their doctor for a DS1500 report. A DS1500 form can be used in claims for attendance allowance, personal independence payment, employment and support allowance, universal credit and disability living allowance. The form will be used by the government office dealing with your benefit to decide whether you can claim benefits under the special rules.
